Sports Physicals & ImPACT Tests

Online Sports Paperwork                                            

Students wishing to participate in athletics during the 2019-2020 school year must submit the following paperwork online at ArbiterAthlete.com no later than Thursday, August 1, at 3 PM; no exceptions.

  • PIAA Physical Form
  • Athlete & Parent Agreement
  • ImPACT Test

If you have not played on a York Catholic athletic team since the online process started last year, ArbiterAthlete accounts must be set-up by each athlete and his or her parent.  Please click here for the instructions on how to set-up an account. If you played on a York Catholic athletic team last year, you already have an existing account.  Please use the same ID and password you used last year.

ALL athletes must upload updated paperwork to be eligible to play during the 2019-20 school year. Doctors’ signatures must be dated AFTER June 1, 2019.

Failure to complete the online paperwork by the deadline will mean the student will not be permitted to practice the first day of the season.  For some sports, this could mean the student will not make the team.  Contact Mr. Josh Cross at jcross@yorkcatholic.org or 717-846-8871 x230 if you have any questions and to be sure you have a valid ImPACT Test on file.  If you need technical assistance with Arbiter Athlete/PlanetHS, email schoolsupport@planeths.com.

Physicals

Please be sure to complete the medical information online (see above) before your doctor’s appointment and only print the 2 pages indicated for the doctor to review and complete.

Note to parents of athletes entering Grade 11:  The Department of Health requires a physical for your child when entering grade 11.  The form used by the DOH is different from the form used by the PIAA.   Please be sure to take BOTH forms to your doctor.

ImPACT Testing

There has been extensive media coverage about athletics and concussions.  York Catholic has been at the forefront in this area and includes the ImPACT Test (Immediate Post-Concussive Assessment and Cognitive Testing) to monitor our athletes.

All interscholastic athletes, cheerleaders, and marching band members are required to take an ImPACT Test to establish a cognitive baseline for concussions. This is an established technology used in order to determine the return to play/activity guidelines for participants who have sustained a concussion.

The test is administered on a computer.  The baseline is adequate for two years. Therefore, if a student participated on a YC team last season and had an ImPACT test, he or she is not required to repeat it this season.

Please email Mr. Josh Cross at jcross@yorkcatholic.org to see if a valid ImPACT Test is on file.  Athletes will NOT be permitted to practice until the test is completed.  It is the athlete’s responsibility to be sure the updated baseline report is on file at YC.  Students MUST check with Mr. Cross to see if the updated ImPACT Test was transmitted properly and is on file.
