Fall Sports Physicals

Fall student-athletes are reminded that a physical is required prior to the start of the fall sports season. York Catholic will be hosting physicals for YC fall athletes on Thursday, June 22, from 5-8 PM at YC.  Cost is $15 with checks made payable to York Catholic.  Appointments must be scheduled by clicking the button below.  

If you prefer to use your primary care physician, now is the time to contact your child’s physician for a physical appointment as some offices do not have availability until 2-3 months from now.  Physicals must be dated after June 1, so be sure your appointment occurs after June 1.

The link to the required forms to take to your appointment (at YC or your physician’s office) will be posted on the YC website on June 1.

Completed physical forms must be uploaded to the familyid.com website by August 2.  Failure to complete the online forms by the deadline will mean the student will not be permitted to practice the first day of the season. For some sports, this could mean the student will not make the team.
